Medical forms and reports

You may need help with completing medical forms or require medical certification or a letter. Courtyard Health Clinic can help with the completion of most medical forms and of course can carry out supporting medicals and tests as required.

We can help you with Power of Attorney, insurance medicals, sports medicals, sick notes, travel medicals, visa medicals and other medical certification and letters.
In many cases, the assessment and form completion can be undertaken during a standard consultation with one of our doctors. Please note that additional charges may apply if further investigations are required, for example, blood tests or imaging.

We kindly ask that you supply a summary of your NHS GP records, along with any other relevant medical correspondence.

Visa medicals

If you are planning to live, work or study abroad, you may need to apply for a visa. But for certain countries, a visa application might also require a medical assessment as part of the application process. The need for a medical examination often depends on the type of visa application and country, for example work, study or residency permits.

Courtyard Health Clinic will help to ensure that your visa application is as smooth and fast as possible.

DVLA medicals

If you require a DVLA medical assessment regarding your fitness to drive, Courtyard Health Clinic offer a range of medical assessments and tests to support this process.

This may be necessary if you have developed a notifiable medical condition or disability that could impact your ability to drive safely. Alternatively, you may be over the age of 70 and are required to submit medical evidence when renewing your driving licence, or you may be seeking to upgrade your licence, for example, to operate a minibus or motorhome.

Please ensure you bring the following to your consultation:

  • The D4 Medical Form
  • Photographic identification (passport or driving licence)
  • Any current medication
  • Relevant correspondence from your GP or hospital, as well as any investigation results related to medical conditions that may impact your driving eligibility
  • Glasses or contact lenses, if worn
